Site Monitoring Report Forms
Site Monitoring Reports are fundamental in understanding the changes and impacts that can be made to a site. As a steward, it is vital that you fill out this report every time you visit your assigned site. Your report is a specific snapshot of the site, so please be as thorough and detailed as possible.
This document records in detail the degree of impact or the lack of change to the sites. Please be thorough and very detailed. You may print off a copy of the Site Monitoring Report prior to your site visit.

Note on abbreviations: MR stands for Monitoring Report.
Photographic Logs
This form is used to document your site-related photographs. The information requested is extremely valuable in documenting where the image came from, and what the image shows. It also helps an Agency Representative locate where you were standing when they try to refind the subject of your photo.
Please keep a log of your photos as you take them so that you can record the subject of the photo and where the photo was taken (GPS coordinates are highly recommended).
